Partners in Health
At Priority Health Care, we proudly introduce the Partners in Health Program, an initiative aimed at expanding our reach and enhancing the quality of care for communities through strategic partnerships with businesses, not-for-profit organizations, government agencies and other healthcare companies.
The Partners in Health Program is designed to provide a seamless, integrated healthcare experience by collaborating with a diverse range of organizations and agencies. Some of the key collaborations include:
Local Department of Health Partnerships: We work closely with local health departments to conduct various public health projects including community outreach, contact tracing, youth development, citizen engagement, and more! Our proactive approach helps in early detection and containment, ensuring the safety and well-being of the community.
Pop-Up Clinics: We organize pop-up clinics for Flu, COVID-19, and other immunizations to make vaccinations more accessible. These clinics are set up in convenient locations, providing easy access to preventive care for the target population (senior living facilities, schools/community centers, churches, etc.)
Supplemental and On-Call Care: Our team is available to provide supplemental care and on-call services to a variety of companies including nursing homes, day programs, and behavioral health facilities, ensuring that they have the support they need to deliver continuous, high-quality care. We work hand-in-hand with our partners to address patient needs promptly and efficiently.
Primary Care Services: We offer comprehensive primary care services tailored to the needs of your constituents (patients/clients/residents/students, etc), including those in behavioral health facilities. Our medical providers focus on personalized care plans that encompass both physical and mental health, aiming to improve overall health outcomes.
By fostering these partnerships, the Partners in Health Program ensures a holistic approach to healthcare, promoting preventive care, chronic disease management, and reducing hospital readmissions. Our goal is to create a healthier future through collaborative efforts that prioritize the well-being of the individuals we serve.
