Follow the steps below to schedule an appointment. Once your appointment has been scheduled, you will receive instructions on how to register for the Care Portal. You will also receive intake paperwork to be completed before your appointment. If you have any questions or need assistance, please give the Care Team a phone call at (888) 341-6401.

Step 2: Complete all intake paperwork prior to your appointment
Once your appointment is scheduled, you'll receive all your intake paperwork online for you to complete quick and easy.
Step 3: Attend your appointment and meet with a licensed medical provider
Whether your first visit is an in-person or a virtual one, we'll be there ready to meet with you and provide you high-quality care!
Step 4: Begin receiving treatment as a Priority Patient!
During and immediately after your visit, any prescriptions will be sent to your pharmacy of choice and your care coordinator will follow-up to check-in periodically.

A Note About Telehealth and Virtual Visits
At Priority Health Care, we embrace the innovation and convenience of digital healthcare. Both "telehealth" and "virtual" visits refer to medical consultations that take place remotely, using technology to bridge the gap between patients and providers.
Our telehealth and virtual visits are conducted using a secure, HIPAA-compliant platform (Google Meets), ensuring your privacy and safety. To guarantee a seamless experience, patients can verify the compatibility of their electronic devices with our platform. We also offer (and in some cases, require) in-person visits as well (see in-person visits below).
In-Person Visits
At Priority Health Care, our primary commitment is to provide accessible and high-quality care. While the majority of in-person consultations occur at our central medical office in Brooklyn, OH, we understand the importance of proximity and convenience for our patients.
Through our strategic partnership with a leading office space provider, our dedicated team has access to a network of private offices across New York and Ohio. This ensures that when an in-person visit is essential, especially for patients with specific conditions or treatment plans (like those on controlled substances), you can receive care closer to home and within your community. We also offer in-home visits in some situations.
